Please provide your own contact details and a brief outline of the support being requested. Do not include diagnoses, medical records, NDIS numbers, plan documents or other sensitive information in this form. We will contact you securely if more detail is needed.

What happens next: We will acknowledge the referral within one business day, discuss consent and support needs, and explain whether Sherpa may be a suitable provider. Sending this form does not create a service agreement.
