What community nursing can include

Support is tailored to the participant’s disability-related health needs and care plan. Depending on the assessment and available NDIS funding, our nursing team may assist with wound assessment and management, catheter and stoma care, diabetes monitoring, epilepsy management, medication support and other clinical tasks delivered in the participant’s home.

How we get started

We begin with a conversation about the participant’s needs, goals, current care arrangements and preferred schedule. Where appropriate, a nurse completes an assessment and works with the participant, family, support coordinator and other treating professionals to understand the required supports. We then agree on a clear care approach and suitable service schedule before support begins.

Nurse-led, person-centred care

Our approach combines clinical oversight with choice and control. Participants are involved in decisions about their care, routines and communication preferences. We aim to provide consistent support, explain each step clearly and raise any concerns through the appropriate clinical pathway.
